We are currently seeking a compassionate and dedicated Registered Nurse Case Manager to join our team at Home Nursing With Heart in Omaha, NE. As a member of our team, you will have the opportunity to provide patient-centered care to individuals in the comfort of their own homes.
Responsibilities:
- Conduct initial assessments and develop individualized care plans for each patient
- Coordinate and provide skilled nursing care to patients in their homes
- Collaborate with other healthcare professionals to ensure the highest level of care
- Educate patients and their families on their conditions and treatment plans
- Provide emotional support and guidance to patients and their families
Qualifications:
- Valid RN license in the state of Nebraska
- Prior experience in home health or case management
- Strong communication and organizational skills
- Ability to work independently and as part of a team
- Commitment to patient-centered care
